Option 1: Cancel in person at your home club
This is the fastest route if your home club is open and a manager is on duty.

- Ask the front desk for the cancellation form.
- Fill it in with your full legal name, EoS scan card number, billing address, and the reason field.
- Ask for a signed copy and an email confirmation before you leave.
Take a photo of the signed form on your phone. If a charge appears 45 days later, that photo plus the email confirmation closes the dispute quickly.
Option 2: Cancel by certified mail
If you cannot get to the club, send a certified, tracked letter. Address it to either your home club's street address or to ABC Fitness Solutions in Sherwood, Arkansas. Keep the letter short and complete.
